OFFICIAL RULES
THE HY-VEE PERKS HUB ULTIMATE TAILGATE PARTY GIVEAWAY SWEEPSTAKES RULES
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE DOES NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW.
1. ELIGIBILITY:
The Hy-Vee PERKS Hub Ultimate Tailgate Party Giveaway Sweepstakes (the “Sweepstakes”) is open only to existing Hy-Vee PERKS and PERKS+ loyalty program members (“Members”) who are legal residents of Iowa, Illinois, Nebraska, Kansas, Minnesota, Missouri, South Dakota, or Wisconsin and are at least eighteen (18) years old (or nineteen (19) in Nebraska) at the time of entry. Employees of Hy-Vee, Inc. (“Sponsor”) and its parent and/or affiliate companies, as well as the immediate family (spouse, parents, siblings and children) and household members of each such employee, are not eligible to enter or win prizes. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations and are void where prohibited. Participation constitutes an entrant’s full and unconditional agreement to these Official Rules and Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes. Winning a prize is contingent upon fulfilling all requirements set forth herein.
2. SPONSOR:
Hy-Vee, Inc., 5820 Westown Parkway, West Des Moines, Iowa 50266-8223, is the sponsor of the Sweepstakes to be held by the Hy-Vee corporate office.
3. SWEEPSTAKES PERIOD:
The Sweepstakes will commence on August 1, 2025 at 12:00 a.m. Central Daylight Time (“CDT”) and end on September 30, 2025 at 11:59 p.m. CDT (the “Sweepstakes Period”).
4. TO ENTER:
During the Sweepstakes Period, to submit a valid entry, eligible Members must use the following URL https://app.viralsweep.com/sweeps/full/4a4bab-208925 to visit the Sweepstakes website (the “Website”). Once on the Website, participants must follow the instructions to complete and submit the entry form, including providing their full name, mailing address, email address, and telephone number. LIMIT: one entry per person per day during the Sweepstakes Period. The device on which each entrant enters the Sweepstakes must have the acceptance of cookies enabled, or any successor technology.
5. WINNER SELECTION/NOTIFICATION:
The potential Prize Winner will be selected by Sponsor during a random Drawing to be held on or about October 1, 2025. Potential Prize Winner(s) will be drawn from among all eligible entries received by Sponsor. Sponsors decisions are final and binding in all matters relating to the Sweepstakes. The Prize Winner(s) will be notified of his/her selection by phone or email within two (2) days of selection as a prize winner and shall be required to accept the prize within forty-eight (48) hours of notification or said prize shall be deemed refused. Odds of winning depend on the total number of eligible entries received during the Sweepstakes Period.
6. PRIZE AND APPROXIMATE RETAIL VALUE:
Sponsor will randomly select five (5) potential Prize Winners. The Prize Winners will each receive a tailgate catering pack containing the following items: one (1) Di Lusso Piccolo Panino Sandwich Tray – 24 Count (approximate retail value of $40.00); one (1) Pick 3 Boneless Wing Tray (approximate retail value of $24.95); one (1) Charcuterie Deluxe Grazer Board (approximate retail value of $75.00); and one (1) 50/50 Fruit & Veggie Tray (approximate retail value of $50.00). The Prize Winners agree to accept all aspects of the prize “as is,” and Sponsor makes no representations or warranties of any kind, express or implied, and expressly disclaims any warranty of title, merchantability, or fitness for a particular purpose. All federal, state, and local tax liabilities are the sole responsibility of Prize Winners. Prize Winners will be required to complete and return an Affidavit of Eligibility and Publicity/Liability release form and an IRS W-9 form, if applicable. Any prize not accepted by the Prize Winners within forty-eight (48) hours of notification will be deemed refused. Prizes are non-transferable and no substitution will be made except as provided herein at the Sponsor’s sole discretion. Approximate retail value of each prize is $189.95. Total approximate retail value of all prizes is $949.75.
7. PRIZE RESTRICTIONS:
No cash redemptions. No transfer of prize permitted. No substitutions, except Sponsor may substitute a prize of equal or greater value at its sole discretion. All costs not specifically stated herein as being awarded are the responsibility of the winner. Prize must be picked up at Prize Winner’s local Hy-Vee store or as otherwise directed by Sponsor. WINNERS AGREE TO ACCEPT ALL ASPECTS OF THE PRIZE “AS IS,” AND SPONSOR MAKES NO REPRESENTATIONS OR WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED. ALL TAXES, FEES, COSTS, AND EXPENSES OF WHATEVER NATURE SHALL BE THE SOLE RESPONSIBILITY OF THE WINNERS.
8. PUBLICITY:
Except where prohibited, participation in the Sweepstakes constitutes the consent of each winner to Sponsor’s and its respective agents’ use of the winner’s name, age, photograph, voice, likeness and/or image, and hometown city and state for promotional purposes in any media, worldwide, without further payment or consideration.
9. DATA COLLECTION:
Sponsor will be collecting personal data about participants when they enter the Sweepstakes. Personal data includes but is not limited to: name, address, date of birth, e-mail address and telephone number. Please review Sponsor’s privacy policy at: http://www.hy-vee.com/company/policy/privacy.aspx.
10. GENERAL CONDITIONS:
By entering, entrants agree to be bound by these Official Rules and the decisions of the Sponsor, which shall be final and binding. Entrants agree to release and hold harmless Sponsor from any and all liability, including personal injury or property damage, arising from participation in the Sweepstakes or use of a prize. Sponsor is not responsible for lost, late, misdirected, or damaged entries. Sponsor reserves the right to cancel or modify the Sweepstakes if fraud, technical failures, or any other factor impairs the integrity of the Sweepstakes.
11. DISPUTES/CHOICE OF LAW:
All disputes arising out of the Sweepstakes shall be resolved individually in the state or federal courts located in Des Moines, Iowa. Entrants waive all rights to claim punitive, incidental, or consequential damages.
12. OFFICIAL RULES REQUESTS/NOTIFICATION OF WINNER:

Requests must be received by October 30, 2025.
